Counselling Agreement
Louise Uys Wellness Counselling
Specialist Wellness Counsellor | ASCHP Registration No. 13473
1. Welcome
Thank you for choosing Louise Uys Wellness Counselling.
Before your first counselling session, please read this agreement carefully. It provides important information regarding the counselling service, confidentiality, professional scope, appointments, payments, cancellations, refunds, personal information and professional boundaries.
All new clients are required to read and accept this agreement before receiving counselling services.
2. Counselling Services
I understand that I am receiving counselling from Louise Uys, Specialist Wellness Counsellor, registered with the Association for Supportive Counsellors and Holistic Practitioners (ASCHP), Registration No. 13473.
Counselling provides a supportive and confidential space focused on emotional wellbeing, personal experiences, relationships, challenges and periods of change.
I understand that Specialist Wellness Counselling does not replace medical, psychiatric, psychological or other specialised professional care. Where my needs fall outside the counsellor's professional scope of practice, an appropriate referral may be recommended.
3. Appointments and Payment
Appointments are confirmed once the required payment has been received.
Clients are responsible for keeping track of their appointment dates and times and attending their sessions as arranged.
4. Cancellation and Refund Policy
Should you need to cancel your appointment, the following refund policy applies:
More than 48 hours' notice: 100% refund
24 to 48 hours' notice: 50% refund
Less than 24 hours' notice: No refund
Missed appointment or no-show: No refund
5. Rescheduling
Appointments may be rescheduled once, provided at least 24 hours' notice is given. The full payment will be transferred to the new appointment.
Requests to reschedule with less than 24 hours' notice will be treated as a late cancellation. The session fee will be forfeited, and a new appointment will require a new booking and payment.
6. Refund Process
Where a refund is applicable, you will be requested to provide the necessary banking details.
Refunds will be processed within 5 to 7 business days after the required banking information has been received. The time taken for the funds to reflect may also depend on the processing times of the relevant financial institutions.
If I need to cancel or reschedule your appointment, you may choose either to reschedule your session or receive a full refund of the session fee.
7. Counselling Agreement and Consent
Acceptance of this agreement is required before your first counselling session.
If you choose not to accept or consent to this agreement, counselling services cannot be provided. Please notify me as soon as possible should you decide not to proceed.
If you have already booked and paid but subsequently choose not to accept the agreement, your decision will be treated as a cancellation and the Cancellation and Refund Policy above will apply according to the amount of notice provided.
8. Confidentiality
Information shared during counselling will be treated as confidential and handled with care and respect.
Information will not ordinarily be shared with another person without your consent. Confidentiality may, however, be limited where disclosure is required or permitted by law or professional obligations. This may include circumstances where there is a serious concern regarding your safety or the safety of another person, or where there is suspected abuse, neglect or harm involving a child or vulnerable person.
Where reasonably possible and appropriate, the need for disclosure will be discussed with you.
9. Professional Consultation and Supervision
I understand that aspects of my counselling may, where professionally appropriate, be discussed confidentially for professional consultation or supervision.
Reasonable steps will be taken to protect my identity, and identifying information will not be disclosed unnecessarily.
10. Personal Information and Records
Personal information voluntarily provided will form part of your confidential counselling record.
Personal information and counselling records will be handled in accordance with applicable South African privacy requirements, including the Protection of Personal Information Act (POPIA).
Information will be collected, processed and retained only for legitimate purposes associated with the provision and administration of counselling services.
11. Online Counselling
Where counselling takes place online, reasonable measures will be taken to protect privacy and confidentiality.
Clients are encouraged to participate from a private environment where they are comfortable speaking freely and are responsible for taking reasonable precautions regarding their own device, internet connection and surroundings.
I understand that electronic communication and online services may carry privacy and technological risks that cannot be completely eliminated.
12. Professional Scope
Louise Uys Wellness Counselling provides services within the professional scope of a Specialist Wellness Counsellor.
I understand that Louise Uys Wellness Counselling does not provide psychological or psychiatric diagnoses, formal psychological assessments, psycho-legal assessments, forensic assessments, evaluative reports or other services falling outside this professional scope.
Counselling services are not provided for the purpose of litigation, legal disputes, custody proceedings or other legal proceedings. The counsellor does not undertake to provide expert testimony, psycho-legal opinions or assessments for use in legal proceedings.
Where appropriate, referral to another suitably qualified professional may be recommended.
